How we get to know you and your pet
We insist on meeting both you and your pet(s) prior to our first engagement. It is important that we meet your pet prior to our first visit while you are away.
Some of the information we need to gather during the consultation includes, but is not limited to:
- Pet’s name & breed
- Pet’s general schedule and routine
- Your travel information
- when you are leaving
- when you expect to return
- where you will be staying
- your emergency contact information
- Name, address, and phone number of your veterinarian
- Name and contact information of a friend or relative in town that we can contact in case of emergency
- Information about who else has access to your home
We will review the agreement and provide you with a Veterinarian Notification Card that authorizes a certain level of treatment (defined by you) in your absence.
